1) SBDM Council is composed of 1 administrator, 4 teachers and 3 parents. Members of the 2011-2012 SBDM Council at Middletown Elementary School are, administrator, Patty Salyer; teachers, Tammy Wagner, Sarah Cunningham, Jenny Gallahue; and parents, Kelly Bowen, Amanda O’Mahoney and Art Thacker. The School-Based Decision Making Council of Middletown Elementary School shall have the responsibility to enhance student achievement through the policies adopted as specified in KRS 160.345. SBDM Council Meeting Dates 2011-2012:

A. Composition B. Eligibility C. Selection Process Parents: All parents (including parents,
stepparents, foster parents, or legal guardians) who have a student(s)
pre-registered to attend or enrolled in the school are eligible to
participate in the parent elections. The PTA Board of Managers shall be
responsible for determining the election procedures, conducting the
election, and notifying the principal in writing of the parents elected to
the SBDM Council. Results must be reported to the principal by July 1 of
each year. D. Terms of Office According to the School-Based Decision Making Law, KRS 160.345 (2) (c). "the school council shall have the responsibility to set school policy consistent with district board policy which shall provide an environment to enhance the students' achievement..." A policy is a written document that guides the educational process at the school. The SBDM Law specifically lists ten areas for policy adoption by the 1. Determination of curriculum, including needs assessment and
curriculum development: 2. Assignment of all instructional and non-instructional staff time 3. Assignment of students to classes and programs within the school 4. Determination of the schedule of the school day and week 5. Determination of use of school space during the school day 6. Planning and resolution of issues regarding instructional practices 7. Selection and implementation of discipline and classroom-management techniques consistent with the procedures established in the Code of Acceptable Behavior and Discipline 8. Selection of extracurricular programs and determination of policies relating to student participation 9. Procedures consistent with local school board policy for determining alignment with state standards, technology utilization, and program appraisal 10. Consultation with the principal for
